Is it asking for the perimeter of the outer line segments (of the rectangles)
excluding the triangle ? I don't understand "heavy line segments" as well.

Doing a bit of reverse engineering (given that we know the answer :-)),

2 * perimeter of rectangles - perimeter of inner triangle
= 2 * 12 - 3
= 21

???

In the diagrams, when a line could be either continuous or dotted, we also refer to the continuous part as heavy. Supposedly the shaded part is dashed and rest is heavy here.
