My doubt : Rule 1) REQUIRE is a verb that takes the either Command subjunctive or the Infinitive.

For ex. We require that HE BE here OR We require HIM TO BE here

Rule 2) Also the correct IDIOM is REQUIRE X to Y.

Experts please advise why B is wrong , it correctly follows the rule 1 I guess?

Also please tell the differences and advise the conditions to use above 2 rule ?

Here, the implication is that hand sanitizers DEMAND that there be 60 percent minimum alcohol concentration.
This meaning is nonsensical.
It is not possible for hand sanitizers to issue a demand.
Also, it is unclear WHERE there must BE a 60 percent minimum alcohol concentration.
Eliminate B.

OA: Hand sanitizers require a 60 percent minimum alcohol concentration to kill most harmful bacteria.
Here, require = NEED.
Conveyed meaning:
Hand sanitizers NEED a 60 percent minimum alcohol concentration to kill most harmful bacteria.
This meaning is sensical.
