This SC has a bit of a quirk to it. The opening part of this sentence is a modifying phrase; it's tempting to think that what immediately follows the comma would be a reference to the wife, BUT the modifier is describing what HENRY dreams about, so the first word after the comma should be a reference to Henry.

Answers D and E can be eliminated for the same reason - they don't match the opening modifying phrase.

in D, did is standing in for appeared, the simple past tense form of appears.
The result is the following:
When Henry dreams, his wife appears to him as she appeared in her youth.
Here, a reader might construe that -- just as Henry's wife now appears only in his dreams -- so too IN HER YOUTH she appeared only in his dreams.
Not the intended comparison.
