Huge Discrepancy in Score- Extremely confused

I just retook a GMATPrep Practice Test and ended up with the exact same score after almost 2 months of studying 580. I was really surprised that I ended up with about the same score for quant as I thought it went well. Verbal for me is still a struggle but I ended up scoring pretty much the exact same for both sections. I have no idea what is going wrong with my studying right now. I think fatigue might have been a factor while taking it today, but I still can't believe that it affected me so much. Can someone give me some advice? Anyone in the same boat?

I have taken practice tests from a variety of sources almost every week as seen below. Honestly was expecting at least something in the low 600 range.

GMATPrep - 580
Kaplan-560
MGMAT- 640
Veritas- 690
GMAT Tutor (Economist)- 690

Today: GMAT Prep 580....

Did you evaluate your errors? You need to see what types of errors you made in order to know where to go from here.

Here are the three (four) categories:

1) Mistakes based on a lack of knowledge - you did not know the formula you did not know the grammar rule etc.

2) Mistake based on strategy or application of the rule - Difficulty in diagnosing the problem.

3) Silly mistakes - errors based on avoidable things, such as answering the wrong question, making an error in calculation, etc. Basically, these are the problems where you say "I should have had that."

If you can break down your mistakes this way you can know what you need to do in order to get a score more like the one that you expect.

Thanks for the advice. Yes, I usually look over what I went wrong and keep an error log. The only reason I can think of right now is the other non GMATPrep CATs were not reliable and I fatigue took a toll.
