Doubt -- 2 options

A report by the American Medical Association indicated that a surge in the number of malpractise suits began about ten years ago and efforts to contain it since then had been partially successful

a. that efforts since then to contain it have been only partially successful
b. that efforts to contain it since then only have been partially successful

Option 2 has a misplaced "only". It seems to suggest that only the efforts since then have been partially successful.

Option 1 is correct.

No "that"

B) that efforts since then to contain it have been only partially successful
Present Perfect Usage "Have been"... the number of suits surged and efforts are still ongoing to contain it.
