Among the facts that contributed to cracks in supposedly earthquake-proof defects in buildings in california there were construction defects like steel columns not being welded properly to other factors, welds that they folded too quickly and with too much heat, and a design whose connection configurations were problematic and unreliable.

In B, they refers to who? It does not refer to any one clearly

Even D sounds a bit weird because it uses past perfect had not been. The other two examples use simple past.

What is the source of this Q?

Amongst the provided 3 choices, Choice D is the best suited:
1) Among the facts ... were (there is not required as in Choice A)
2) such as should be used since steel columns is an example of contruction defect in this stmt.
3) To maintain parallelism : "welds that they folded.." and "a design whose connection " - they is not required. In this statement emphasis is on weld as weld is described in more detail; when they is placed emphasis is on they.
