My recommender forgot some questions! Problem or not?

Hi all,

My recommender forgot answering some questions in the letter of recommendation. Furthermore, the format of the letter was not correct. Hence he contacted the school to remove the previous letter and replaced it by a new one.

I would like to know what school will think about this? Is this a problem with my application?
