Possible long-term consequences of the "greenhouse effect," the gradual warming of the Earth's climate, may include melting the polar ice caps and a rising sea level.

A. may include melting the polar ice caps and a rising sea level
B. may include the melting of polar ice caps and the rising sea level
C. may include polar ice caps that are melting and sea levels that are rising
D. include melting the polar ice caps and sea levels that are rising
E. include melting of the polar ice caps and a rise in sea level

I guess the answer is B as it follows the rules of parallelism and looks more apt.
Even the other options does not fit in as good as B.

What is the OA??

It has to be E. Possible and may look two redundant words.
First take: 640 (50M, 27V) - RC needs 300% improvement
Second take: coming soon..

Does "Melting" is acting as a gerund here?
If yes only then the parallelism has been maintained!!

Melting is most definitely a Gerund here.

For example look at the following examples to cast away that sneaking suspicion.

1. The polar ice caps are melting quickly. -PP qualifying 'polar ice caps'

2. Melting of polar ice caps has quickened recently. -- Gerund being qualified by the verb 'to be'.

If Melting is acting as gerund here...which seems to be, what is the main verb in the sentence? Is it 'Include' ?

Yes it is , Include is main verb here.

this sentence will be something like after omitting appositive

Possible long-term consequences of the "greenhouse effect" include melting of the polar ice caps and a rise in sea level.

underline portion also ||el -noun....noun- in case of preposition we can assume "of" and "in" ||el.
