D is out of sync with above pattern. D OUT

a consumer reporting agency is singular and requires "its"
AB are out.

B, D - out - incorrect idiom
As this is subjunctive mode, hence the idiom should be requires that
A - out - Wrong subject verb agreement - "Their" incorrectly used for consumer reporting agency
E - out - Wrong subject verb agreement - "It" incorrectly used for consumer reporting agencies

C it is. Rightly uses "requires that xy" and also it's is used properly to refer back to agency
