Here two things are important.

1. Parallelism
2. Subject-Verb agreement.

For, first thing, we need the THAT after the conjunction AND.
So, option (A) and (D) are ruled out.

For, second thing, we need the either the plural subject or something that agrees with the verb EVALUATE
Since the subject [independent panel] is singular, hence you need the auxilary verb WOULD to meet the subject-verb agreement.

Hence, rule out option (B) and (C).
