If you are using despite then it should be followed by noun. If Of construction is required then in spite of is preffered.

"extreme" is an adjective, not a noun!

Despite can be followed by a noun, pronoun, verb (in "ing" form), adjective or adverb. So no, it doesn't have to be followed by a noun.
