A is correct because it is logic to say that

insurance plan imposing and requiring

and that

doctor to see... and to spend

2 couples of paralele

imposing and requiring

to see and to spend

Insurance plan cannot spend time with each patient..

So it should be D

parelism b/w impose and require

Spending is just the modifier for Doctor.

Plans that impose

1) stricter limits on medical services
2)require doctors to see more patients

If you write Plans that impose spending less time with each it gets unlogical.

We need that to explain what's is this plan ? So A,B out C is out because all of them paralel.
