What 'they' refers to in (E) is ambiguous.

'they' can refer to therapies or patients.

there is also an issue with punctuation

Main clause , coordinating conjunction(or)+main clause

or Item+0+coordinating conjunction(or) +Item

the second one is correct for this case

Core idea is : Some patients ...................... have received inadequate treatment

To describe what inadequate treatment is the author uses having ................................................ ( he gives two examples or Items )

Answer is A

I have a general question that when to use 'have been' and when 'having been'. what's the difference between two?
