Historians have long debated whether the spectacular rise and fall of Napoleon in the decades after the French Revolution was the necessary outcome of the political vacuum having been created by the toppled monarchy.
A) was the necessary outcome of the political vacuum having been created by the toppled monarchy
B) was the necessary outcome of the political vacuum created by toppling the monarchy
C) were the necessary outcome of the political vacuum that the toppled monarchy created
D) was the necessary outcome of the political vacuum created from toppling the monarchy
E) were the necessary outcome of the political vacuum created by the toppling of the monarchy

1.Can anyone explain whether Rise and fall should function as singular or plural?
2.Moreover isnt the spectacular modifying both rise and fall here?
3.Should not it be outcomes in all the option if rise and fall were plural.

well i think 'rise and fall' looks like a single verb at first glance as it is in rhythm but i think it is combination of two different verbs which can exist independently
so Rise and Fall should be plural
for eg playing and eating are good habits.
so A,B,D out

Between C and E i think both are wrong
I agree with your third question,it should be outcomes now
I think people will still dig reasons for it but i personally think both are not gramatically correct.

For the sake of gmat to get next question,if you have to choose between two,i will chose E as
i think C changes the meaning of original sentence (reserves the cause and effect)
