The answer choice E uses the correct construction so [adj] that.
Moreover the first 3 options that begin with lack is illogical.they tell that the doctors who lack transportation.
It should have been, that because of the lack of adequate transportation in rural areas, doctors find it difficult to distribute themselves throughout a countryside that is becoming increasingly populated.

In A and C, them is at best ambiguous and at worst incorrect. Them seems to refer to doctors. When the object is the same as the subject, a reflexive pronoun must be used: doctors [find] it difficult to distribute THEMSELVES. Eliminate A and C.

In B, becomes is incorrect. Increasingly populated implies an ongoing, continuous action. Thus, the correct wording is becoming increasingly populated. Eliminate B.

In D, are lacking so much in adequate transportation is not idiomatic. The correct idiom is X is so lacking in Y. Eliminate D.

The correct answer is E.
