Can you please explain the solution for this with explanations?

A recording system was so secretly installed and operated in the Kennedy Oval Office that even Theodore C. Sorensen, the White House counsel, did not know it existed.

E is out because it is passive.
D is out because it is a sentence fragment.
C is out because the "it was" construction is awkward.
B is out because it needs "that".
