What is wrong with option A[/spoiler]
Generally, an introductory prepositional modifier serves as an ADVERB modifying the verb in the following clause.
In A, in a previous design (an adverb pointing to THE PAST) cannot serve to modify how the weight is concentrated (in the present).
Eliminate A.

In B, it (subject pronoun) seems to refer to the weight (subject of the preceding clause), implying that the WEIGHT is now lined with lead.
The intended meaning is that the DISCUS is now lined with lead.
Eliminate B.

In D, the discus (subject) lacks a verb.
The result is an incomplete sentence.
Eliminate D.

In E, the discus was...now lined makes no sense.
A present tense verb is needed: the discuss IS now lined with lead.
Eliminate E.

The correct answer is C.

The answer should be 'C.'
The reason I dismissed 'A' was because the noun modifier should always touch the subject. The "it" in but now it is lined should directly link back to the discus which is not the case here. It doesn't help me answer the question ' what is lined with lead'?
Same for B.
C: No misplaced modifiers. I can easily understand that we are talking about the discus here.
D: Sounds just plain awkward
E: Lacks parallelism
