why not D?

With D, the sentence would be:

Beneath the soil of the Malheur National Forest in eastern Oregon, a fungus that has for centuries slowly woven its way through the roots of trees and has become the largest living single organism known to humans.

It is not really a complete sentence. The "skeleton" of the sentence with option D would be:

...a fungus that..., and has become...

Basically the and in sentence above is redundant.
