IMO C is the Best choice. Insist indicates the subjunctive mood and that must be followed by base verb.

I choose C.

a - no 'should' in subjunctive (insisted that)
b - 'should be paying' is a no-go
d - no 'should' in subjunctive (insisted that)
e - wrong modifier with 'author'
