Ideally, the modifier is intended to modify the whole clause . Asset allocators do their job with the intention of providing good results.

B is wrong for couple of reasons : which refers to portfolios but it is not intention of portfolios but of aset allocators.

D and E are not worth considering even.

Betwen A and C : C is the right answer if it modifies portfolios as ed modifier will play role of noun modifier.
A is correct if it plays role of adverbail modifier because it is an prepositional phrase and can be used as an adverbial modifier. But the phrase does not sound too appropriate, i stick to C

Since there is major confusion between Choices A and C, I will only focus on A, C in my solution.

Original Sentence - Choice A
Asset allocators create portfolios{,...,} with the intention to turn in good results.
Meaning - In this choice, the modifier "with the intention to..." modifies the action of asset allocators. It provides the reason why asset allocators created certain portfolios. Notice that the portion of the sentence within commas - often in the form of mutual funds, describes the "portfolios".
Errors - This choice has idiom error. The correct idiom is "with the intention of turning".

Choice C
Asset allocators create portfolios {,...,} intended to {turn in...}.
Meaning Now in this choice, the modifier has been changed to "intended to..." and it appropriately modifies portfolios. The sentence conveys the intended meaning of the sentence. Asset allocators created certain portfolios. These portfolios were intended to turn in good results. Thus in essence, the intention of asset allocators to turn in good results is met.

Please note that if Choice A is worded with correct idiom, it will be correct:
Asset allocators create portfolios with the intention of turning in good results.
