Choose (A)

(A) became the first spaceship to carry
CORRECT.

(B) was the first spaceship carrying
Wrong tense. It is still the first spaceship that carried humans for the moon trip.

(C) was the first spaceship that carried
Wrong tense. It is still the first spaceship that carried humans for the moon trip.

(D) is the first spaceship that has carried
Wrong tense. Currently it is not carrying any human being.

(E) became the first spaceship, carrying
Comma after spaceship distorts the meaning of the sentence. Now it means that those metallic pieces carried the men instead of spaceship.

Why E is wrong? Doesnot Participle Phrase modifies the closest Noun. In this case 'spaceship'?

Because 'carrying' implies a continuous tense and the action of the sentence has already happened in the past.
