In A, B and C what "which" and "their" refer to is not clear.

In D also use of "their" is ambiguous.

E is the best option.

E seems to be the right answer..

In A,B, and C .. not sure about the "which" reference.

In D, their can refer to either patients or antagonists.

"Patients, which may limit" means patients may limit as which modifies the immediate preceding noun. The intended meaning of sentence is to say that such an effect will limit and this is done by D\E.

there are multiple plural pronoun hence their in D is Incorrect. Hence E should be correct answer
