Restrictive/Non Restrictive Clause

1) School's performance is gauged by an index, such as standardized test scores, that are called objective.

What does THAT refers to here. Does it refer to index (in that case it should be that is) or test scores?

The above sentence is wrong for two reasons:

1. "that" cannot refer to "scores" because no comma is used between the NOUN and that.
2. NOUN + That + verb: --the verb MUST agree with the NOUN.


What is the source of this sentence?

Source of this question is Kaplan Premier program 2009 Edition.

As per the book, that in this sentence actually refers to index and correct answer is

School's performance is gauged by an index, such as standardized test scores, that is called objective.

My doubt is whether that can be used to refer to the noun that is separated from it as it is in the above example.
