Most vaccines are derived from weakened or killed strains of the same
virus that they prevent, unlike the derivation of smallpox vaccine
from a different virus altogether.

(B) smallpox vaccine, which is derived from a different virus altogether

(C) smallpox's vaccine, which gets derived from a different virus altogether

My guess is "B"
It is a parallel structure: we are comparing "most vaccines" to one vaccine, which leaves us with "B" or "C".
Then I choose B, because of parallel between "are" and "is". Also "smallpox's" sounds weird.

B IMO

Vaccine should be compared to vaccine, So A, D, E out...

In C, apart for possessive smallpox's, it also says.. gets derived.. which can mean it gets derived on it own.. which is not true..
