You need simple past "led" here as the action has occurred in the past.

B and C use incorrect "have led"
D and E use "There were concerns" and doesn't use an appropriate "and"
to connect two phrases.

Is it A ?

I would also go with (B).

I don't know about (A) since when you read the entire statement (A) ending in "to serve metropolitan Boston" ... sounds incomplete.

Answer please?

D and E sound very awkward, especially D , and in E it says "there were concerns leading" concerns about what? when in the original sentence its clearly stated that its concerns about public health. Hope that helps
