(B) year and thus -- can clearly see that people who lose the workdays ---cost the nation $50 billion in medical expenses and lost

(C) year, so as to cost --awkward sentence construction---the nation $50 billion in medical expenses and lost

(D) year that --what? year?---costs the nation $50 billion in lost medical expenses and

(E) year, which thus cost --even worse than C---the nation $50 billion in lost medical expenses and

B seems to be the answer

First of all 2/3 split of cost/costs..which one is correct?
Which leads us to the question:
What causes the loss? The Migraine or the Americans or the workdays?
I think that "thus" is fundamental--it would be incorrect to say that migraine cause these problems directly; "thus" means something akin to "in this way," so we have a kind of explanation of how migraine causes this problem.
So.B seems correct..but HOLD ON!Should'nt it then be COSTS???
Hmm..SO now from the costs/cost pt iof view correct ans should be D??

It is clear who starts a subordinate clause modifying Americans affected by migraine, removing the extra's:

Migraine afflicts perhaps 18 million Americans, who collectively lose 64 million workdays a year, and they cost the nation $50 billion in medical expenses and lost work time.

So it is clear two things are told in subordinate clause vis lose 64 millions workdays a year and cost the nation $50b in medical expenses and lost work time

These two should be expressed in parallel structures.

(A) year, and they cost the nation $50 billion in medical expenses and lost
they has not antecedents, only subject is who
(B) year and thus cost the nation $50 billion in medical expenses and lost

(C) year, so as to cost the nation $50 billion in medical expenses and lost
so as to starts another clause and distorts the meaning.
(D) year that costs the nation $50 billion in lost medical expenses and
again that has a bad referrent only singular referrent are Migraine and year none of them suitable here.

(E) year, which thus cost the nation $50 billion in lost medical expenses and
which modifies year which is wrong

hmm.

the old "paper test" questions are often rather subpar.

in this case choice (b) is still the *best* option -- which i suppose is the only criterion that matters at the end of the day -- but it's weird: it states that the 18 million americans themselves cost the nation $50 million. for the sentence to be totally logical, it should state instead that migraine costs the nation this amount of money.
