Should I disclose mild ADHD?

I was diagnosed of mild ADHD when I was young. I have no problem living with it, I've graduated good school, had a pretty good career for the last 4 years. I want to know whether I should disclose this in my essay(or optional), because the disease really played a large influence of my life(i.e. long term goals). I didn't when I applied for Columbia...now other R1 schools coming, I am wondering if I should.
