A.
B. but then suffered a second fatal attack
C. when she suffered a second and fatal attack
D. and then suffered a second fatal attack
E. when a second fatal attack was suffered

OA is C

what is the difference between A and C

A suggests both attacks were fatal
C states more clearly the second attack was the fatal one

A suggests that both second and first were fatal, which is impossible because 'fatal' means bringing death...

C suggests that she suffered a second attack which was fatal...
