A. despite they need it badly
B. despite they badly need it
C. in spite of they need it badly
D. however much they need it
E. therefore, they need it badly.

Tricky one!!

But still IMO B

A. despite they need it badly (inappropriate as the adverb is in the end)
B. despite they badly need it (CORRECT)
C. in spite of they need it badly (same reason as for choice (a), as "In spite of" = "Despite")
D. however much they need it (altered intent)
E. therefore, they need it badly ("therefore" plays spoilsport)



Would really appreciate comments.

My doubt:
As, in spite of = despite (correct me if I am wrong)..this makes choice (A) & (C) same. So, the better choice is (B)

another vote for B, in A badly (adverb) modifies noun "medical attention" which it can't, in B badly modifies need (verb) which it can.

Guys, the OA is D. I really don't know how D is right. The question is from Barrons GMAT book.

I am not a big fan of this question's quality, but, of the options presented, D seems to be the most befitting.

The question is trying to contrast between the need and the real situation. It means to tell you that Howsoever big be the need, the reality does not permit its fulfillment....
Now again read the question, D does this to a larger and clearer extent than does any other option. 'Despite' would be awkward to use in this sentence.
