The intended meaning is that the low-end insurance plans will led to stricter limits on medical services. Also, the doctors will be forced to see more patients, hence, the doctors will be spending less time with each patient on an average.

Only D maintains this meaning. The important thing to note is that we need 'Spending' here because the doctor is spending less time WHILE he is attending the patients.
