If by authentic you mean real or truthful, then the answer is yes. Everything and anything in your applications must be the truth and not something you make up or exaggerate. Schools have started investigating applicants reported data, so you are running a risk of reporting fraudulent information. In the event you are found to have committed fraud, you will be automatically rejected or have an offer of admission rescinded.
If by authentic you mean documentation of the achievement, then no, that is not necessary. However, the achievement should be something that can be verified if the school contacted the school, company or organization that awarded/presented the achievement.
