Which one/all of the following is correct?

1. The new law requires that the government is accountable to the Lokpal.
2. The new law requires that the government be accountable to the Lokpal.
3. The new law requires the government is accountable to Lokpal.
4. The new law requires the government be accountable to Lokpal.
5. The new law requires the government to be accountable to Lokpal.

2 and 5 are correct usages.
"require that x be("be" can be replaced with any base form of the verb) y" and " requires x to do("to do can be replaced with any infinitive) y" are the two correct usages

Not correct. While "technically", both "require that" and "require to" might be correct, in this case, "require that" is the correct one. The reason being that the new law does "not" require the government; it requires something "from" the government.
