One more for 'E'.
I'm really old, but I'll never be too old to become more educated.

Theory:

1. Deaths and other actions approved====> Means deaths have been approved. Can we approve DEATH?
2. THAT Vs Which:

To define something, we need to use THAT after the noun that we want to define.

Actions which ---> is wrong. It should be "actions that".

3. Meeting in May, XX--means XX meets in May.

Now apply:

The options A, B, and C can be eliminated on the basis of #1. The theory number 2 applies to the options B and C.
The option D is out for number 3.

Answer is E.

E seems best.

ABC&D all make it sound like deaths were approved by the BOD, which makes no sense.

That leaves E as the only one that does not do that as it separates the actions approved by the BOD from the reporting od deaths.

I would chose E because it looks like active voice - The July 12 issue reports xxxx
but A is like xxxxx will be reported in July 12 issue.

GMAT prefers active voice comared to passive

We have its in almost every option..
So, we should have a proper referent for its. Logical referent should be company and in none of the options we have company mentioned..
its refers to company PAPER...
at its May meeting -> at company PAPER meeting???
