Before anyone replies to this post and eliminates threat to health on grounds of wordiness, I would like to bring to the knowledge of all that MGMAT SC guide says prepostional phrases that begin with prepositions other than OF shouldnt be reduced to noun adjectives .
So threat to health shouldnt be reduced to health threat

I could breakdown this to 2 and 4

(2)
child obesity has escalated exponentially and is recognized today as a real health threat

(4)
child obesity has escalated exponentially and is recognized in today's world as a real threat to health
Will choose 2, as TODAY seems better than IN TODAY'S WORLD(wordy)
Please correct me if am wrong!

i received a message concerning this question.

what is the source of this problem?
all five of the answer choices are very poorly written, and so discussing the problem is not worthwhile (except perhaps to discuss why it's such a horrible problem). however, an identification of the source would be valuable, so that the source can be treated with suspicion in the future.
Ron has been teaching various standardized tests for 20 years.

if you don't know or remember the source of a problem, then don't use the problem!
