Correct : "Only " restricted only to "heart disease and cancer." -it should be place as close as possible.
"Dibetes" singular ......we need ranks and not rank

(8) rank as the nation's third leading cause of death, only surpassed
rank - incorrect

(e) has the rank of the nation's third leading cause of death, only surpassed
rank- incorrect

(D) are the nation's third leading causes of death, surpassed only
are -"dibetes" cannot be plural

(E) have been ranked as the nation's third leading causes of death, only surpassed
"only surpassed" -- only incorrectly placed

D is wrong because are is plural.ELIMINATE

E - why change the tense unnecessarily when simple present tense works!

C is wordier

B is wrong coz of rank.

I'd go with A

Diabetes is singular here..

along with.. together with.. in addition to makes noun singular..

and makes plural

Hence (A)
