A)The largest trade deficit on Earth of any country is that of the United States, with other nations, such as China and Japan, consequently holding stores of US dollars increasing each year.
,prep phrase is necessarily adverbial.
How is the largest trade deficit on Earth of any country that of the United States?
with other nations consequently holding stores of US dollars increasing each year.
...not making much sense.
Clause A and Clause B means both clauses are mutually exclusive: there is no relationship between them.
If, however, there exists any relationship, you insert a phrase or an adverb ,somewhere after and, that communicates such relationship appropriately.
There is a relationship of cause and effect between two facts-
Clause A and consequently Clause B
Clause A and thus Clause B
Clause A and as a result Clause B
all of the above are appropriate constructions to communicate such relationship and thus is the one in OA.
