The answer is D. My question is by reading D it seems like its saying a random jazz pianist and composer thelonious Monk..... Shouldn't it say... Jazz Pianist and Composer, Thelonious Monk.... because now we know that the jazz pianist and composer are modifying the monk, where as in D it seems to be two separate people... Thanks everyone!

A) Thelonious Monk, who was a jazz pianist and composer, produced a body of work both rooted
- both is wrong here, produced a body of work is singular. Also, I think we need to use appositive here, but need expert comment here.

(B) Thelonious Monk, the jazz pianist and composer, produced a body of work that was rooted both
Same as A

(C) Jazz pianist and composer Thelonious Monk, who produced a body of work rooted
It creates a run on sentence. Thelonious Monk, who ........, yet in many ways he stood apart from the mainstream jazz repertory.

(D) Jazz pianist and composer Thelonious Monk produced a body of work that was rooted
(E) Jazz pianist and composer Thelonious Monk produced a body of work rooted both
Both is wrong here. Idiom is, Both X and Y, where X and Y must be parallel. "Both in the stride-piano tradition of Willie (The Lion) Smith AND Duke Ellington" - certainly not parallel.
