Surveys have shown that in up to 40 percent of elderly people living independently in affluent countries, the consumption of one or more essential nutrients is insufficient or they have deficient levels of these nutrients in their blood.

I could not trace "they" in B. I think that "they" refer to people.

Question: What is the actual mistake in B?

A, B , C can be thrown out because of un-idiomatic usage of in upto. This leaves you with D and E. E has the right usage of OR. Hence E
"Single Malt is better than Blended"

Question: What is the actual mistake in B?

B can be simplified as follows:

"Surveys have shown that in xyz number of elderly people, people consume ..."

This sentence is non-sensical since it states that "in people, people consume..."

Let me know if this makes sense

The problem with B is modifier issue.

See,

Surveys have shown that in up to 40 percent of elderly people, they consume ..... [ wrong modifier ]

the sentence can be written as :

Surveys have shown that in up to 40 percent of elderly people, the consumption of nutrients ........
