IMO the answer is E

The word "possible" at the beginning of the sentence makes "may" redundant. Eliminate solutions a,b,and c.

Between D and E, the answer has to be in passive voice to emphasize the consequences. Solution D can be interpreted that the "consequences" cause melting, when logically the melting of the ice caps and rise in sea level need to be that examples of "consequences."

IMO A

Only A and C are parallel.
C: "may include polar ice caps that are melting and sea levels that are rising "

long-term consequences include melting of .. and rising of .. and not polar ice caps and sea level themselves.

What is OA?

OA is E
I took C as answer for parallel construction. thanks paual for pointing out "possible" makes May reduntant. if thats taken into account than automatically E is best choice.
