I think it should B instead of A.

In subjunctive & imperative mood we use

verb + that + bare infinitive

C - it's = it is incorrect

D - essential you maintain incorrect

E - essentially you maintain incorrect

Am I missing something?

IMO it is B.
In the subjunctive mood, "require that" is appropriate.

Also "saferguards" should be read as "safeguards"
- a typo in translation !!

If you are seeking parallelism as an explanation, I'd have to differ on that

For parallelism to come into play, the answer should have been

its value is gone so it is essential to maintain proper saferguards within your business and to require all employees to sign[/quote] - but then the sentence doesn't make sense.

"essential .. to and require ....to" have to be in parallel - I don't quite get the subjunctive issue here. The ans should be A.
