(singular "operation" disagrees with plural "providers" unless the providers have merged into one operation)

have transitioned into sustainable, locally-grown operation
(same agreement problem as A, also missing the indefinite article "a")

have transitioned into locally-grown, sustainable operations
(works well; agreement problem fixed)

are transitioned into sustainable, locally-grown operations
("are transitioned" is awkward and incorrect)

are transitioning into locally-grown, sustainable operation
("are transitioning" could be okay if it is an ongoing process, but we still have the same problems here that we have in B)
