IMO B

A. falling behind is not correct here because it detoriates the main meaning, that the reason for being unable to handle the increase is the payment to doctors and hospitals.
B. For me it is correct.
C. "in its paying" is awkward. unidiomatic.
D. no need -ing (proving) here. Past tense is ok.
E. "paying in" is less preferable to "payment to". Also if there was "it" in the sentence, meaning would be more clear. Now it is hard to get the right meaning, what proved unable to handle the increase--> the metropolitian area or the company?

Looking for OA.
Thank you.
