How far in advance of the deadline should apps be submitted?

If an app is submitted a few days before R1 deadline, and it turns out something is missing (GMAT score didn't come through, rec didn't come through, etc), do the schools give you a chance to add in the missing info/still consider your app for R1, or do you automatically get bumped to R2 time period?

It probably depends on the school. Just make sure you submit a completed application and then you won't need to worry about it. There are ways to review it to make sure everything is in there.

For recs there is a small grace period but if your recommenders submit them late, your whole application will be held back. It's your job to manage the process and business school admissions committees think that it shows poor management skills to not get it in. So make it happen!

For GMAT score, you self report for most schools so you just need to have it done by the cutoff and then GMAC will allow you to upload the PDF of the score report.
