it should be A, i think

B uses "was", there is no need for change of tense. sentence structure implies that the instances of past are compared to present.

C - the sentence does not require the use of present perfect, because it would include the instances of obesity present now.

D and E are out because the non underline portion contains "more", so use of "than" is justified.
