Created in 1945 to reduce poverty and stabilize foreign currency markets, the world bank and international monetary fund have, according to some critics, continually struggled to meet the expectations of their major shareholders- a group comprising many of the world's rich nations-but neglected their intended beneficiaries in the developing world.

Please explain.

Also please explain why did we choose BUT instead of WHILE in this. Can this be also a reason to eliminate options. If yes or no please explain.

In B and D, that lacks a clear antecedent.
Eliminate B and D.

Appositives are two nouns that appear side-by-side, with the second serving to explain or define the first.
C and E: their major shareholders' expectations - a group comprising many of the world's rich nations
Here, a group seems to be in apposition to expectations, implying that EXPECTATIONS are A GROUP COMPRISING MANY OF THE WORLD'S RICH NATIONS.
This meaning is nonsensical.
Eliminate C and E.

The correct answer is A.

Generaliy, while + VERBing serves to express CONCURRENCE.
B: struggled...while neglecting
D: had to struggle...while neglecting
In these options, the usage of while neglecting implies that the bank and fund were neglecting AT THE SAME TIME as they struggled/had to struggle.
Not the intended meaning.

The intention here is to express not concurrence but CONTRAST, as in the OA:
continually struggled to meet the expectations...but neglected their intended beneficiaries.
Here, the usage of but conveys the following contrast:
Although the bank and fund struggled to do something POSITIVE (meet the expectations), they did something NEGATIVE (neglected their intended beneficiaries).
