Despite the recent election of a woman to the office of prime minister, the status of women in Pakistan is little changed from how it was in the last century.
(A) is little changed from how it was
(B) is a little changed from how it was
(C) has changed little
(D) has changed little from how it has been
(E) is little changed from the way it was

OA after some discussions
Getting defeated is just a temporary notion, giving it up is what makes it permanent.

Should be C. No need to add from x to y thing after it is referring to change and the period "last century" is already mentioned.

What is the OA?

Action coming into the present so we would g with has.

Option D is wrong because of the usage of has been.

So option C is the answer and this also happens to be the shortest and to the point.

IMO C

The catch word "recent" in the non underlined part demands for a present perfect tense. (unless other parts of the sentence indicate change in time frame).Eliminate A,B and E.

(C) has changed little
(D) has changed little from how it has been (you cannot use present perfect tense to describe a previous incident in this case)
