When he phones her, she tells him to stop calling, but he acted as if he had not understood her.

a.she tells him to stop calling, but he acted as if he had not understood her.

b. she told him to stop calling, but he acted as if he had not understood her.

c. she tells him to stop calling, but he actes as if he did not understood her.

d. she tells him to stop calling, but he actes as if he has not understood her.

e. she tells him to stop calling, but he acted as if he does not understand her.

A changes the tense of the sentence

B when he phones her, she told him... is incorrect, it has to be she tells him.

C did not understood is incorrect grammar

D is correct in tense and grammar

E again changes the tense of the sentence.
