If I keep the sense same as intended in the original question, what is the correct GMAT rule for the conjunction in this case:

Let me take another example. What is the correct answer in the following:

She came and started playing.
She came, and started playing.

Only the first one is right here.

We use comma + and construction
1.when we have three or more items in a series
2.when we are to connect two independent clauses.

The second one is wrong for both of the above reasons.
