Conclusion: using this test, doctors can largely avoid unnecessary removals of the appendix without, however, performing any fewer necessary ones than before since.....
Inference: Using this test if doctors are able to 'largely avoid unnecessary removal' that mean all the diagnose, which this test say +ve are the ones that indeed require removal.
I think C say the same.

For me it was between B and C, and I chose B. If the doctors are performing ALL the necessary procedures without performing any fewer procedures, so the ones they are not performing are in fact those that should not have been performed in the first place.
