If the school gives a recommendation form, but says the recommender may write his/her own letter, which option is preferable (form VS letter)? Does the letter need to address the same questions as the form or can it be "free form"? Thanks!

If the school gives you both options, they are truly giving you both options. Your recommender can fill out either the form or the letter. And yes, if the recommender decides to use the letter option, s/he should be sure to cover all the topics in the form.
